The Details   of LES EA: Laboratory Administrative Amendment

What you need   for LES EA: Laboratory Administrative Amendment

Anyone (private corporation, individual, partnership or association, or other entity) who intends to conduct analytical measurements for the purposes of determining compliance with MassDEP standards where the MassDEP or the laboratory’s client requires such measurements be conducted by a certified laboratory must apply for certification from the Department.

This application may be used to

  • Drop certification for analyte(s)/method(s)
  • Request updates to information on file with the MassDEP for the laboratory (e.g., address, phone number, facilities, equipment, or personnel

NOTE: To be considered for certification for additional analytes or methods, please complete an LES 03 EA (Chemistry) or LES 04 EA (Microbiology) application.

What is needed to apply:

  • Current MassDEP Laboratory Authorization Number and EEA ePLACE account.
