Instructions for registered manufacturers for modifying a previously submitted disclosure report.
Submitted disclosure reports will be reflected in the online reporting system after the regularly scheduled data refreshes. These take place monthly.

What you need   for Modify a previously submitted disclosure report for the Pharmaceutical Code of Conduct

To  modify a disclosure report you must submit a new, complete disclosure report including all covered recipients for the calendar year. We will remove the previously submitted report and replace it with this new file.

You must submit reports via email (pharmameddata@massmail.state.ma.us) in an electronic file, with specific formatting required by the Department.

 

How to submit   Modify a previously submitted disclosure report for the Pharmaceutical Code of Conduct

Once you create the modified disclosure file in the correct format, email the file to the Department:

  1. Submit the files as an attachment via email to: Pharmameddata@massmail.state.ma.us
  2. All emails containing disclosure report submissions must contain the following information in the subject line of the email:
    • "Manufacturer name", "manufacturer unique ID", and the phrase "Disclosure Report Modification for year XXXX"
      • "XXXX" would be the year the data in the disclosure report refers to
