What you need for Register a LASER device
You may register a LASER device for the following uses:
- Medical/Dental
- Veterinary
- Academic
- Manufacturer (i.e., make & sell lasers)
- Industrial (i.e., non-medical use)
- Entertainment (e.g., laser light show)
- To register a LASER device for entertainment use, please submit a copy of your valid FDA and/or FAA variance with your application
- Dealer / Distributor (i.e., sell lasers)
LASER Safety Officer (LSO) qualifications
Your designated LSO must submit qualifications to include education, training, and/or experience for new registrations or LSO change.
