How to report

Reportable incidents must be reported by the Residence to the Assisted Living Certification Unit within 24 hours of the incident.

Incident Reports should be made using the Dynamics Incident Reporting system. Access to Dynamics will be made available during the certification process of your ALR. Questions or requests for access should be sent to ALRIncidentReport@mass.gov
