What you need   for Request a criminal record self-audit

To request a self-audit, you must:

You can make a request to self-audit at any time. If you make your request at least every 90 days, there is no fee. If you make an additional request within the 90 day period, there is a $25 fee. Additional requests made within the 90 day period need to include a money order for $25, made payable to the Commonwealth of Massachusetts.

To submit your request by mail, send your completed and notarized form to:

If your form is not completed and your signature isn't notarized, we will not process your request.
