What you need   for Request CORI As An Individual

To register for an online iCORI account, you must have a valid Massachusetts driver's license or I.D. card. 

If you do not have a Massachusetts Drivers License. or I.D. please read below on how to submit a request by mail

Name Fee Unit
Personal CORI request $25 per request
Open access CORI request $50 per request
Affidavit of indigency $0 per request

How to request   Request CORI As An Individual

Individuals with a valid Massachusetts drivers license or I.D. can register online to use the iCORI system to receive their personal CORI.

Individuals who do not have a Massachusetts driver's license or I.D. card must submit their CORI requests by mail. The Personal Request Form can be downloaded here and will need to be mailed into the address listed on the form. 

  • This includes attorney requests for clients who do not have a Massachusetts drivers license or I.D. card.
