Please note: As of 6/23/25 you will no longer be required to fill out the paper attestation, you will attest in the application. You will be required to upload the following:
If you are Active Duty, you will be required to upload your current Active Duty orders.
If you are the Spouse of an Active-Duty Service Member who has left employment in another state to join your spouse, you will be required to upload your spouse's current transfer orders to the Commonwealth with your name listed as a dependent, and/or marriage certificate.
If you are a Veteran or have prior military service, you will be required to upload your DD214 form OR other proof of U.S. military service.
