How to file Request VALOR Act benefits - OLRH
To request benefits under the VALOR Act, you must identify yourself by completing and signing the applicable affidavit form below:
- Affidavit of Military Service Status
- Affidavit of Military Spouse Status
- Affidavit of Veteran Status
Submit online:
Please use the eLicensing system to upload the appropriate affidavit and supporting documentation to your new application or by beginning an amendment to your current registration and/or certification.
