If you need help requesting a verification of licensure for a licensee, please visit our guide. The manual is also available in DOCX format for download.

Before starting this application, please visit the Massachusetts Health Professions License Verification Site at https://checkahealthlicense.mass.gov and search for the license/registration you would like to request a verification of licensure for.

Most licensees will be able to download the verification of licensure immediately and for free from the Health Professions License Verification Site.

Only start this application if you were unable to generate the verification of licensure from the Health Professions License Verification Site. This application will be processed manually by the Bureau of Health Professions Licensure, and has a non-refundable $15 processing fee.
