What you need
The Department of Unemployment Assistance (DUA) mails 1099-G forms by the end of January for the previous year. For example, if you collected unemployment benefits in May 2025, you will get a 1099-G in January of 2026.
If you did not receive one or you need an additional copy, you can request it online or by phone.
How to request
Forms for the previous year will be available by the end of January of the year after you collect benefits.
To download your 1099-G:
- Log in to your Unemployment Services for Workers account
- Select Additional Services
- Find the box titled 1099 and use the link to get your form
