What you need
Starting May 6, 2025, UI Online will completely transition to Unemployment Services for Employers as the single system to manage both your tax account and employee benefit claims in one place.
- DUA will no longer send or accept fact-finding requests by mail. You will need to respond to claim requests online through the Unemployment Services for Employers system.
- You'll log in to Unemployment Services for Employers using your MyMassGov business account.
How to respond
- Log in to Unemployment Services for Employers
- On your Dashboard, select Action Center to see fact-finding questionnaires and other requests for information
- Follow the system prompts to complete and submit the information. You can submit documents and information online to support your questionnaire.
Next steps
After you submit your questionnaire
Your questionnaire is reviewed and if more information is needed, you will receive another request for additional information from DUA. Generally, you will receive this request by phone.
Once all information is reviewed, a determination will be issued. Please note that whether or not you are entitled to a copy of the determination (and if you will receive appeal rights) depends on the timeliness of your response and the adequacy of the information you provided on the questionnaire.

Submitting your questionnaire after the deadline
You may still submit your questionnaire after the deadline has passed. Please note that submitting documents after the deadline may affect your rights to the claim.
In your Unemployment Services for Employers account Dashboard, select "Request to response to past due fact filing" under the Account card
Viewing saved and submitted questionnaires
You can access all saved and submitted questionnaires from your Unemployment Services for Employers account. Select Additional Services and then Search submissions.
