Complete application in accordance with 105 CMR 120.132 “Expiration and Termination of Licenses  and Decommissioning of Sites and Separate  Buildings or Outdoor Area”.

How to submit

Email your completed and signed certificate to: DRCRadMaterials@mass.gov
