Submit a Radioactive Waste Survey

The Division of Radiation Control tracks the nature and quantity of Low Level Radioactive Waste produced in the Commonwealth each year by requiring all licensees to complete a survey.

  • Radioactive Materials license number
  • Licensee name
  • Radiation Safety Officer
  • Contact information
  • Radioactive waste generated, storage, and disposal information in calendar year (Class A, B, C, or HVLA)
  • Waste minimization statement/plan
