What you need for WS 06: Registration of a Class V UIC Well and Modification of an Existing Registration
Class V wells are used to inject non-hazardous fluids into the ground and require registration by MassDEP. Check the types and descriptions of these wells at: UIC Class V wells to determine the category of well you are registering.
You will need to complete a Technical Compliance Form and Certification Statement and attach it to your registration application. These forms are used by MassDEP and the applicant to determine whether a UIC registration for a proposed discharge may be approved. Technical compliance forms are not required if you are registering an existing UIC well for the purpose of closing the well.
If you are applying to register multiple wells, a separate registration application form must be submitted for each facility address. Multiple wells may only be registered under one application if all wells are on the same property. Also, all wells must be in the same municipality (with some limited exceptions).

Applications to register new UIC wells or existing UIC wells that are not currently registered must be submitted through the eDEP electronic filing system. Submission of an application on paper is no longer accepted.

UIC Application for Closed-Loop Ground Source Heat Pump Well
As of December 2, 2016, the installation and operation of a Class V closed-loop or direct exchange Ground Source Heat Pump (GSHP) well no longer requires the filing of a UIC Registration application with MassDEP, provided that the well has been installed and is operating in accordance with MassDEP’s Guidelines for Ground Source Heat Pump Wells, and provided that the well is not used to produce water. This change is based upon amendments to the MassDEP Ground Water Discharge Permit Program regulations, 314 CMR 5.00, that were promulgated on that date.
