In order to add a Member(s) to a Business Entity's Producer License, please print and complete the following form and mail to the Division of Insurance with the total licensing fee of $75 per member to:

Please note that any member you may wish to add to your Business Entity Producer License must be licensed individually as a Producer in Massachusetts.
In order to delete a Member from a Business Entity's Producer License, please email the following information to ProducerUpdate.Mailbox@state.ma.us:

- Member Name(s)
- Member(s) National Producer Number(s)#
