How to schedule an in-home vaccination

Call the In-Home Vaccination Central Intake Line at (833) 983-0485.  

  • CDR Health, which operates the call center, can schedule appointments and deliver in-home vaccinations Monday through Friday from 9 a.m. to 5 p.m.  
  • Representatives are available in both English and Spanish, and also have access to translators for over 100 other languages

You can schedule an appointment directly on the CDR Health Patient Portal page at patientportalma.com.

Follow these four steps: 

  1. Create an account or if you already have an account, log in.
  2. Select get vaccinated.
  3. Register for your visit.
  4. Present your code during your scheduled visit.

What to expect

In-home vaccinations will be:  

  • Scheduled within a week of request   
  • Performed by fully vaccinated medical professionals, following all public health safety guidelines   
  • Tailored to meet the individual needs of the individual, as discussed on the scheduling call   

Pfizer and Moderna vaccines will be routinely offered to all individuals 6 months and older. Novavax vaccine is also available upon request when booking the appointment.
