COVID-19 In-Home Vaccination Program

In-home vaccinations are available for anyone who has difficulty getting to or using a community vaccination location.

In-home vaccinations are available for anyone who has difficulty getting to or using a community vaccination location, including children.

How to schedule an in-home vaccination

  • By phone: Please call the In-Home Vaccination Central Intake Line at (833) 983-0485. 

    • Representatives from CDR Health, which is operating the call center, scheduling appointments, and delivering in-home vaccinations are available at this number Monday through Friday from 9 a.m. to 5 p.m. 

    • Representatives are available in both English and Spanish, and also have access to translators for over 100 other languages 

  • Online: You may also schedule an appointment directly on the CDR Health Patient Portal page at patientportalma.com 

In-home vaccinations will be: 

  • Scheduled within a week of request  

  • Performed by fully vaccinated medical professionals, following all public health safety guidelines  

  • Tailored to meet the individual needs of the individual, as discussed on the scheduling call  

  • Pfizer and Moderna vaccines are available.  Novavax vaccine may be available for special situations.dividuals 6 months and older will be offered the Pfizer or Moderna vaccine.

    • People ages 6 years and older who are unvaccinated or previously received only monovalent vaccine (original vaccine) doses are recommended to receive 1 bivalent (updated) mRNA vaccine dose (Pfizer or Moderna) 

    • Most people ages 6 years and older who are not moderately or severely immunocompromised and have received 1 dose of an bivalent (updated) mRNA vaccine (Pfizer or Moderna) do not need any further vaccine doses at this time. People ages 65 years and older who received 1 dose of a bivalent vaccine have the option to receive 1 additional dose at least 4 months after the first bivalent dose.

      • People ages 12 years an older who previously received 1 or 2 monovalent Novavax COVID-19 primary series dose(s) are recommended to receive 1 bivalent mRNA vaccine dose. People ages 18 years and older who received the Janssen COVID-19 Vaccine primary series dose are recommended to receive 1 bivalent mRNA vaccine dose (Moderna or Pfizer) at least 2 months after completion of the primary series dose.  l 

  • Novavax may be offered to individuals 12 years and older in certain limited situations.
