CPA Limited Liability Company: LC

  1. Certificate of Regulatory Board Request a License Verification or Certified License History Document: Public Accountancy.
  2. Certificate of Good Standing issued and sealed by the Secretary of the Commonwealth, Corporations Division.
  3. Certificate of Insurance or Capital Program as per Board of Accountancy Rules and Regulations.
  4. Photocopy of all members’ current licenses.
  5. Firms with non-CPA owner(s) include a written statement including the individual’s name(s), the date of admission as a partner or member and any information regarding a professional or vocational license or the individual’s right to practice a profession or vocation. For further requirements, 252 CMR 4.

CPA Limited Liability Partnership: LP

  1. Certificate of Regulatory Board Request a License Verification or Certified License History Document: Public Accountancy.
  2. Certificate of Good Standing issued and sealed by the Secretary of the Commonwealth, Corporations Division.
  3. Certificate of Insurance or Capital Program as per Board of Accountancy Rules and Regulations Photocopy of all members’ current licenses.
  4. Firms with non-CPA owner(s) include a written statement including the individual’s name(s), the date of admission as a partner or member and any information regarding a professional or vocational license or the individual’s right to practice a profession or vocation. For further requirements, 252 CMR 4.

Business Corporation: BC

  1. Certificate of Good Standing issued and sealed by the Secretary of the Commonwealth, Corporations Division.
  2. Certificate of Insurance or Capital Program as per Board of Accountancy Rules and Regulations.
  3. Photocopy of all members’ current licenses.
  4. Firms with non-CPA owner(s) include a written statement including the individual’s name(s), the date of admission as a partner or member and any information regarding a professional or vocational license or the individual’s right to practice a profession or vocation. CPA Partnership: P

  1. Certificate of Good Standing issued and sealed by the Secretary of the Commonwealth, Corporations Division.
  2. Certificate of Insurance or Capital Program as per Board of Accountancy Rules and Regulations.
  3. Photocopy of all members’ current licenses.
  4. CPA Corporation: C

  1. Certificate of Good Standing issued and sealed by the Secretary of the Commonwealth, Corporations Division.
