DBA Information

All Business Entity names must be approved by the Division of Insurance. Business Entity names must be professional sounding and must not be the same as another entity's name. A Business Entity may have one DBA name.

All Individual Producers are required to have the word, "insurance" in their DBA names. Individuals seeking to use a DBA name must first have the name approved by the Division of Insurance.  Individual DBA names may not include the words "group", "associates/association", "partnership" or "company".  An individual insurance producer may use only one DBA name.
