Eligibility
In-home vaccination is available for people (including children) who have difficulty getting to a community vaccination location, particularly those facing mobility or transportation challenges. There is no cost to the patient (note that if you have insurance, they will be billed for the cost of the vaccine).
How to schedule an in-home vaccination

Call the In-Home Vaccination Central Intake Line at (833) 983-0485.
- CDR Health, which operates the call center, can schedule appointments and deliver in-home vaccinations Monday through Friday from 9 a.m. to 5 p.m.
- Representatives are available in English and Spanish and also have access to translators for more than 100 other languages

- Create an account or if you already have an account, log in.
- Select get vaccinated.
- Register for your visit.
- Present your code during your scheduled visit.
What to expect
- Due to requirements of the funding streams that support the in-home vaccination program, patients must receive a COVID vaccine in order to receive an influenza vaccine. Patients may not receive only an influenza vaccine.
- COVID: Pfizer and Moderna vaccines will be routinely offered to all individuals 6 months and older. If it is available, Novavax may be requested by individuals 12 years and older.
- Flu: If it is available, the high-dose flu vaccine may be requested by individuals 65 years and older.
- Appointments are tailored to meet the individual needs of the individual, as discussed on the scheduling call
- In-home vaccinations are performed by fully-vaccinated medical professionals, following all public health safety guidelines
