Massachusetts has a long-standing commitment to ensuring that older adults and people with disabilities have access to community-living opportunities and supports that address each individual’s diverse needs, abilities and backgrounds. In 2008, the Commonwealth honored this commitment with the establishment of the Community First Olmstead Plan. This plan created the framework for our Community First policy agenda and an action plan for the future of Long Term Services and Supports (LTSS) in Massachusetts.

Since 2011, the Money Follows the Person (MFP) Demonstration Rebalancing Grant has worked to advance our state’s “Community First” commitment to ensuring that older adults and people with disabilities have access to the supports and services needed to live with dignity and independence in the community. To date, over 2,151 individuals have transitioned from nursing facilities and long-stay hospitals to community-settings through the Massachusetts MFP Demonstration. 

As detailed in the MFP Demonstration Sustainability Plan, the Commonwealth will continue to support transitioning individuals to the community following the conclusion of the MFP Demonstration Rebalancing Grant. The MFP Demonstration Grant has allowed Massachusetts to develop several new Demonstration services for MFP consumers transitioning back to the community. These services will, for the most part, continue to be offered through Home and Community-Based Services (HCBS) waivers or potentially as elements of new opportunities under the Affordable Care Act. Equally important, Massachusetts ADRCs will continue their commitment to transitioning individuals from facilities when transitions supported under MFP end in December 2016.  The ADRC member agencies have played a crucial role in the MFP Demonstration, serving as key stakeholders and contracting as MFP Transition Entities. The Massachusetts Aging and Disability Resource Consortia (ADRC) Five-Year Strategic Plan serves as a testament to the network’s commitment to transitioning individuals from nursing facilities and long-stay hospitals to the community.

The Executive Office of Health and Human Services remains committed ensuring that older adults and people with disabilities have access to community-living opportunities and supports. We look forward to working closely with our ADRC partners as they continue their mission to transitioning individuals from facilities when transitions supported under the MFP Demonstration end in December 2016.
