These are forms required in order to gain access to MOSES, AppStream and AWS.

All Employees - Required form for accessing MOSES, AppStream & AWS

• MOSES/AWS/AppStream Request Form 2022 (Fillable PDF)
• MOSES/AWS/AppStream Request Form 2022 (PDF for Print)

The MOSES/AWS/AppStream Request Form must be completed and signed and submitted to the email address shown on the form ([e-mail]). The required Policy documents must be signed and retained at your office.  

EOLWD Policy Requirements for both EOLWD and Non-EOLWD Staff

THE FOLLOWING SIGNED DOCUMENTS MUST BE UPDATED ANNUALLY AND MAINTAINED AT THE EMPLOYING OFFICE AND BE AVAILABLE FOR REVIEW BY INTERNAL CONTROL AND DCS QUALITY ASSURANCE

For EOLWD Staff, a copy of the annual Policy Review signoff sheet from EOLWD HR can be maintained in lieu of the ITR Policy and the Confidentiality Policy.
