Program Description
Commonwealth of Massachusetts ("we," "us," or "our") offers a text messaging (SMS/MMS) program that sends appointment reminders to users who opt in.
How to Opt In
You may opt in to receive text messages from us by Signing the opt in SMS form and submitting it with your retirement application or requesting to opt in verbally on the phone when speaking to PERAC staff and a confirmation text will be sent for you to confirm you opted in t.
By opting in, you consent to receive recurring automated appointment text messages from Public Employee Retirement Administration Commission at the mobile number provided. Consent to receive texts is not a condition of any purchase.
Message Frequency
Message frequency up to 4 messages per medical appointment scheduled.
Message and Data Rates
Message and data rates may apply. Charges will vary based on your mobile carrier and plan. Contact your carrier for details about your text and data plan.
How to Opt Out
You can cancel the SMS program at any time. Text STOP to the number you received messages from. After you send the SMS message "STOP" to us, we will send you an SMS message confirming that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, sign up as you did the first time, and we will start sending SMS messages to you again.

Supported Carriers
Carriers are not liable for delayed or undelivered messages. Participating carriers include AT&T, T-Mobile, Verizon, and most major US carriers. Carrier coverage may vary and is subject to change without notice.
Eligibility
You must be at least 18 years old (or the age of majority in your jurisdiction) and a resident of the United States to participate in this SMS program. By opting in, you represent that you meet these requirements and that you are the account holder for the mobile number provided, or that you have the account holder's permission to opt in.
Privacy
Your privacy is important to us. Please review our SMS Privacy Policy and general Privacy Policy for details on how we collect, use, and protect your information. We do not sell, rent, or share mobile opt-in information or phone numbers with third parties for their own marketing purposes.
Disclaimer of Warranties
Our SMS program is provided "as is" without warranties of any kind, either express or implied. We do not guarantee that messages will be delivered in a timely manner or at all, as delivery is subject to effective transmission from your wireless carrier/provider and factors outside our control.
Limitation of Liability
To the fullest extent permitted by law, Public Employee Retirement Administration Commission and its affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of, or inability to use, the SMS program.
Changes to These Terms
We reserve the right to modify or discontinue the SMS program, or update these Terms and Conditions, at any time. Changes will be posted on this page with an updated "Last Updated" date. Continued participation in the SMS program after changes are posted constitutes your acceptance of the revised terms.
Governing Law
These Terms and Conditions are governed by the laws of the State of [Your State], without regard to conflict of law principles.
