Three steps to find a One Care plan

Plans are organized by county. For this reason, you must find your county before you can choose a plan.

Step 1: Know your county

Look up your city to find your county.

Go to this web page:  City-to-County Finder: StatsAmerica.

Select MA in the drop-down list and then type your city in the blank box. Once you type your city into the box, click Submit and you will see what your county is.

For example, if I type in “Easthampton,” I learn that I am in Hampshire county.

 

Step 2: Find plans in your county

Look at the lists of counties for each plan below. Find which plan or plans list your county.

For example, in step 1 I learned Easthampton is in Hampshire county. Based on that information, I can choose from either Commonwealth Care Alliance or United Health Care.

Step 3: Read about the plans

Go to the website of each plan you can choose from. If you have questions about the plans, call them. Check the list of questions under this table.

  • Is the doctor I have now in the new plan?
  • If not, who will be my doctor?
  • Which hospitals can I go to?
