Type of Report

Schools Required to Submit a Report
Monthly Activities Report CSHS-funded districts and nonpublic schools
Status Report CSHS-funded districts and nonpublic schools
BMI Report All public school districts with grades 1, 4, 7, or 10
(CSHS-funded districts and nonpublic schools should include these data with their Status Report, not as an additional data submission)
SBIRT report All public school districts with a middle or high school
(CSHS-funded districts should include these data with their Status Report, not as an additional data submission)
Epinephrine Administration Report All schools in which epinephrine was administered in school
Medication Error Report All schools in which a medication error occurred
Workforce Development Grant Quarterly Progress Report Districts and nonpublic schools funded through the Workforce Development Grant
