Type of ReportSchools Required to Submit a Report
Monthly Activities ReportCSHS-funded districts and nonpublic schools
Status ReportCSHS-funded districts and nonpublic schools
BMI ReportAll public school districts with grades 1, 4, 7, or 10
(CSHS-funded districts and nonpublic schools should include these data with their Status Report, not as an additional data submission)
SBIRT reportAll public school districts with a middle or high school
(CSHS-funded districts should include these data with their Status Report, not as an additional data submission)
Epinephrine Administration ReportAll schools in which epinephrine was administered in school
Medication Error ReportAll schools in which a medication error occurred
Workforce Development Grant Quarterly Progress ReportDistricts and nonpublic schools funded through the Workforce Development Grant
