News  New Training & Education Program Application

The Board of Certification of Community Health Workers is excited to announce that it is now accepting applications for the Community Health Worker Training and Education Programs.

 The Community Health Worker Training & Education Program Application is now available.  

Kindly complete the application in its entirety and include all supporting materials.  Please mail the completed application and all requested supporting materials to the following address:

Please be aware that only complete applications will be reviewed by the Board. If there is pertinent information missing, the Board’s staff will notify applicants that the application is incomplete and cannot be reviewed by the Board until it is complete. 

 Once complete, it may take 4-8 weeks for the CHW Board to review the Training and Education Program application contents and make a determination whether to accept or deny the application.    

 Please note: At the present time, there are no applications fees required, however, fee requirements are subject to change in the future
