Discharge planning should begin at intake and must meet the requirements outlined in the DPH Hospital Transfer and Discharge Guidance and all requirements related to discharge planning outlined in DMH regulation 104 CMR 27.00.
Discharge planning should also include all involved parties including communication with health insurance plans, family/friends, and any case managers or service providers connected with the patient
