What is the SIS-A®, 2nd Ed. Assessment?
The Supports Intensity Scale-Adult Version®, 2nd Edition Assessment (SIS-A®, 2nd Ed.) is a tool used to identify support needs for an individual with an IDD or Autism. It helps to provide a complete view of the individual and what they need to engage in everyday activities and meet their goals.

How to Participate
DDS is currently piloting the assessment with a random sample of individuals receiving services.
If you have not been contacted to participate, but are interested, fill out the preference survey. If you are eligible, a DDS scheduler will reach out to schedule a SIS-A®, 2nd Ed. assessment.
The Path Forward project supports the Department of Developmental Services’ goal of fully integrating the SIS-A®, 2nd Ed. tool to provide a more consistent, transparent, and equitable way of assessing the support needs of individuals who access services.
