Massachusetts Disability Retirees will soon be receiving their Annual Statement of Earned Income or 91A forms in the mail.
If you are required to submit your 91A form to the Public Employee Retirement Administration Commission (or PERAC), the process is even faster now with our secure online portal, PROSPER, available at mass.gov/91A.
To log in, you will need the unique barcode that will be on the form you receive in the mail and to answer a few questions to verify your identity.
Once you are logged in, you will answer the same questions that are on the paper form and upload the required tax documents to the secure online portal.
All uploaded documents MUST be in PDF format. We cannot accept any other type of file, including image files. If you have your taxes prepared electronically, you should already have these documents available to you as a pdf.
Only have paper copies and no scanner? If you have a mobile phone or tablet, you’re in luck! You can quickly scan the documents yourself by following just a few steps.
For iphone/ipad users:
- Open the Files app and navigate to (or create) the folder where you want to save your document
- To start the scan, tap the three dots icon (or More Button) and select scan documents.
- Position the phone camera over the document and let it auto scan or press the shutter. Continue scanning all the pages until you reach the end of the document, then select save.
Android/Samsung users
- Open the “Google Drive” app on your phone, tap the Camera (or +) icon, select “scan document” and snap an image of the document you want to save. ‘
- Continue scanning pages until you have scanned all the pages in your document, then select save. You will be prompted to name your document, then press “done” and your document is now saved to your google drive
Continue saving all the tax documents you need in this manner then upload when completing your online 91A Form. Note that while our online form is secure, PERAC cannot be responsible for the security of your personal devices. Please ensure you are taking the necessary precautions to protect the personal information on your devices.

The deadline to submit your form is April 15th. Please contact our office immediately if you do not receive your form by the end of February.
