- Mandatory notification from Elevator Inspector Joseph Janis, EM-005665, of an inspection INS-805304, for State ID 281-P-489, which was cited, pursuant to 524 CMR 1.13(7), for safety issues, specifically there being a hole in the floor for access to a sub secondary machine space below the elevator machine room with a ladder going down into the hole, but no handles are provided. Inspector Janis informed them that he is requiring handles be installed for safe ladder access down into the sub machine space, pursuant to 524 Code Mass. Regs. § 1.13(2).

After a Quasi-Judicial Closed Session was held on April 28, 2026, the board requested that a new solution be proposed within a deadline of no more than sixty days which is June 29, 2026, keeping within the following specifications and guidelines:
- accept a modification necessary to install a code compliant 9-inch ladder adjacent to the elevator guide rail and
- installation of a secondary pit mounted stop switch adjacent to the elevator guide rail.
The petitioner was before the board on April 7, 2026, and the board voted to defer the case to a future Quasi-Judicial Closed Session for additional review.
The petitioner is back before the board after previously being before the board on January 27, 2026, when the board voted to place the item on a 60-day hold, which provided a deadline of March 28, 2026. The applicant was requested to gather more information including pit dimensions, plan view, equipment information and demonstration of hardship. New documentation submitted March 27, 2026, included two design drawings and a photograph.
The petitioner was seeking a safety-equivalent variance from 524 CMR - A17.1 – 2013 Section 2.2.4.2 regarding the ladder installation for units 293-P-285 and 293-P-286.
