CMS Five-Star Rating for Nursing Homes:

o January 2026

o Overall Quality: 3 stars

o Health Inspection: 4 stars

o Quality Measure: 3 stars

o Staffing: 1 star*

PBJ reporting issue related to staffing

Pinnacle Report

o 98% favorable rating with an average score of 4.84out of 5

o Communication continues to be the focus area

o Looking into alternatives to recordsatisfaction ratings and real time information that is generated in house.

HPPD:

o Total HPPD was 6.12 for January 2026

o RN 0.93

o LPN 0.8

o CNA 4.38

o Organic staff 94.6%

CHE-OLYMPICS:

o The Rehabilitation Department worked in conjunction with the Facilities Department to build a rink.

o Residents participated in chair hockey and indoor curling.

o Approximately sixty residents participated in the event

There were no questions on the Executive Director’s report, and it was accepted by the Board of Trustees.

Dr. Jed BarashMedical Director Report
  • Posted openings for a Nurse Practitioner and Physician II, with interviews planned over the next few weeks.
  • Collaborating with WellSky and Trident to address and optimize several issues related to laboratory ordering workflows and the presentation of test results within the electronic medical record system.

Question: Ms. Hale asked how many residents are state retirees? Do they have a means to use Tri-Care, is there a long-term care plan?

Answer: Tri-Care is for outside providers

Scott ConsaulFacilities Department Report
  • Work on the elevator sprinkler project in process
    • Most of work was completed 8:00pm – 4:00am to minimize the impact on operations
  • The vendor is awaiting a pre-action cabinet
    • Once the cabinet is received, the project should be completed by late March
  • Owner/Architect/Contractor (“OAC”) meetings are ongoing, and the work is moving forward according to schedule
  • Incinerator Building work is ongoing.  Construction has been slowed due to severe cold weather and recurrent snowstorms.
  • SNF Bathroom Doors:
    • DCAMM and Consigli are still awaiting delivery of three new doors to complete the punch list.
  • QMH Demolition with DCAMM and Consigli:
    • On March 2, 2026, Consigli will be taking over complete control of the site and restricting access behind the Quigley Building to construction personnel. 

Question: Does DCAMM and Pennrose still hold meetings with the residents?

Answer: Yes, every few months.

We will check the date of the next Town Hall meeting when they are due to present and let the Board know if they would like to attend.

Mr. Lyons complimented Mr. Silvestri for doing a good job with the grievance process.

January 2026 Recruitment Totals

  • Current Onboarding positions

Ms. Hale complemented Human Resources for the number of Registered Nurses they have recruited and the culture and climate at the Home.

Mr. Lyons wants to advocate hiring veterans to work at the Home.

Mr. Zografos said the Home is looking to expand opportunities to hire veterans.

Mr. Lyons will share the contact for MDSOA who handles the Boston -North Shore area. Ms. Hale offered to contact the women’s veterans network and the Army Nurse Corps. Organization.

  • 2025 DPH Annual Survey Plans of Correction (POC)are Ongoing. One POC outstanding.
  • 5 Star Rating Focus for Quality Measures. Hospital and ER tracking began.
  • Preparing for the 2025 VA Survey POC.
  • Audit for Excellence Program began in February to include multiple departments conducting audits.
  • Rolling out a Fall Program in conjunction with Clinical Reporting in efforts to reduce falls and maintain Veteran safety.

Ms. West highlighted the newly implemented “Audit for Excellence” program as well as the “post fall huddle” program which helps to identify a root cause of a fall.

  • 9 Tours were completed
  • 13 Grievances investigated:
    • 11 in the SNF
      • 2 in the DOM
  • 3 Residents met with Veterans Legal Services
  • 2 residents met with the VA representative for service-connected disability claims
  • 4 residents admitted to the DOM
  • 2 residents admitted to the SNF
  • Social Services has compiled a list of free tax preparation resources in the Chelsea area and are assisting residents with gathering required tax documents.

Due to the new American Disability Act statewide guidelines, all 45 monitor slides and the newsletter templatewas updated to comply with the 4/24/26 deadline.

o Newsletter highlights:

o New magician entertainer in the home was well received

o Pastor Joel is holding monthly memorial services in the SNF

o Employee Engagement Committee hosted the 2nd International Food Festival

o In progress:

o Editing of two brochures: one for the skilled nursing facility and one for the domiciliary

o Onboarding three volunteers

There were $12466.00 in disbursements from the Legacy Fund for January 2026

  1. $1805.00 for barber services.
  2. $6750.00 to Paul Revere Transportation for buses for resident outings.
  3. $1450.00 to performers for musical entertainment.
  4. $2441.00 to Resounding Joy for resident music therapy services.

Question: Is Springtime when there is someone who comes to give an overview of the accounts?

Answer: We will reach out to the Treasurers’ Office and see if someone can attend next month’s meeting.
