Community Engagement Partner Assessment

This assessment is designed to elicit objective feedback on the levels of Community Engagement throughout the CHNA/CHIP process from the perspective of community partners.

This assessment has 10 sections and will take approximate 5-10 minutes to complete.

Completion of this form is voluntary; however, it is necessary for the Applicant to demonstrate compliance with DPH's Community Engagement Guidelines (PDF) | (DOC) and partner feedback is an integral component to that.
