What you need

To apply, you need to:

  • Complete and sign the entire online application form (found in the links below)
  • Attach a copy of your most recent inspection report issued by the responsible state of local agency

Please submit a separate application and fee payment for each facility seeking a license.

Fees

Pay by credit card or electronic fund transfer using the Environmental Health eLicensing Portal.

How to apply

Go to the Environmental Health eLicensing Portal  and create an account, or log into your existing account. If this is your first time using this new portal, please visit BCEH eLicensing System User Guide.
