To apply, you will need to:

  • Complete and sign the entire online application form (found in the links below)
  • Only complete this form if you are applying to use Dogs and Cats in research and education. No other animals need to be listed as part of this application form.

Please submit a separate application and fee payment for each  institution seeking licensure.

Pay by credit card or electronic fund transfer using the Environmental Health eLicensing Portal.
