The Details
What you need
Complete the Mammography Facility License Application online using the Environmental Health eLicensing Portal and once your application is approved and all fees are paid, an inspection will be scheduled.
To apply through the eLicensing Portal, you will need the following:
- Inspection Contact List
- Name, Title, Phone #, Fax #, email address, mailing address for the following people:
- Facility Inspection Contact
- Owner of Facility
- Most Responsible Individual (i.e. CEO, President, Owner, etc)
- Billing Contact
- Responsible (Lead Interpreting) Physician
- Name, Title, Phone #, Fax #, email address, mailing address for the following people:
- Personnel
- Name, Position/Credentials, MA License # (or registration #), License/Registration expiration date for the following personnel:
- Interpreting Physicians
- Mammographers
- Medical Physicists for Review Workstations (Monitors)
- Medical Physicists for on-site equipment
- Consultants/Service Providers
- Name, Position/Credentials, MA License # (or registration #), License/Registration expiration date for the following personnel:
- Equipment
- Manufacturer
- Unit Model
- Serial #
- Date of Manufacturer
- Date Unit is in use for patients
- Date of Installation
- Date ACR materials submitted
- Date ACR approved
- Current Medical Physicist Survey to upload
- Shielding Design Plan
Adding Facility Personnel for Facility Renewal
If your Diagnostic Radiography or Mammography facility has a significant number of staff to update, streamline the process by downloading the Facility Personnel Template. This template allows you to efficiently input information for all personnel, including but not limited to Medical Physicists. Enter the required details, such as first and last names, license numbers, and license expiration dates for each staff member.
Facilities are encouraged to include as many staff members as possible during the renewal process. If a renewal application cannot be completed before the facility’s expiration date, use this template to submit the personnel details available at that time. Additional personnel can be added later by submitting an amendment to your application through the eLicensing system.
Once the template is completed, upload it to your application following the provided system instructions. For further assistance, consult the user guide or contact our support team.
How to apply
Go to the Environmental Health eLicensing Portal and create an account, or log into your existing account. If this is your first time using this new portal, please visit BCEH eLicensing System User Guide.
