What you need   for Apply for a Mammography facility license

Complete the Mammography Facility License Application and one Mammography Machine Identification Form for each mammography unit being licensed. 

You may submit this packet to the RCP by mail, once your application is approved, you will be invoiced a new-license fee, and an inspection will be scheduled.
