What you need for Register a Mammography machine
Complete one Mammography Machine Identification Form for each mammography unit being licensed. Each Mammography Machine Identification Form must also include the summary pages of the Health Physicist's evaluation of that unit. You may be invoiced a new-unit fee if it is not replacing a unit that is being returned or going out of service.
To remove a Mammography unit:
Change the Responsible Physician, change ownership of the facility, or any other changes pertinent to your Mammography Facility License, please notify this office in writing.
