What you need   for Apply for a Motion Picture Operator license

The filing of this application does not permit an applicant to have charge of or to operate a moving picture machine. A license must be issued by the Office of Public Safety & Inspections (OPSI) and received by the applicant prior to operation.

All of the following items MUST be submitted with this application in order for the application to be processed properly. Failure to submit all required information along with appropriate fee will result in unnecessary delays.

  • Applicants MUST be at least 18 years of age.
  • Completed application with accurate home mailing address and social security number. Application must be completed in ink (see downloads below).
  • 2” x 2” photo, unless use of RMV Photo is authorized.
  • Non-refundable application processing fee.

All fees are non-transferable and non-refundable. Payment must be submitted in the form of check or money order, made payable to the Commonwealth of Massachusetts

Exams

Exams take place the last week of every month.

For example, if your application is received and processed in January, you will sit for an exam in February, and so on. You will be notified of your exam date by mail approximately 2 weeks before an exam. Please visit the OPSI website @ www.mass.gov/dpl/opsi under the Engineering Exam Schedule for additional information regarding the exam.
