The Details
What you need
You must submit all of the following with your renewal application. Missing items or payment will delay processing.
- A completed Motion Picture Operator Renewal Application with your correct home address and Social Security number (must be filled out in ink).
- One 2” x 2” photo, unless RMV photo use is approved.
- Non-refundable application fee.
Important:
- If you don’t renew by the expiration date, your license will be void. After one year, you’ll need to apply again and may have to retake the exam.
- Renewal notices will be sent by email. Be sure to include your email address on the application.
Fees
All fees are non-transferable and non-refundable. Payment must be submitted in the form of check or money order, made payable to the Commonwealth of Massachusetts. Do NOT mail cash.
Write the license number on the front of the check or money order.
