The Details

What you need

All of the following items MUST be submitted with this renewal application in order for the application to be processed properly. Failure to submit all required information along with appropriate fee will result in unnecessary delays.

  • Completed application with accurate home mailing address and social security number. Application must be completed in ink (see downloads below).
  • 2” x 2” photo, unless use of RMV Photo is authorized.
  • Non-refundable application processing fee.

Licenses not renewed by the expiration date shall become void and shall after one year be reinstated only by a new application and re-examination of the licensee if required. All future renewal notices will be sent by E-Mail, please specify email address in the application.

Fees

All fees are non-transferable and non-refundable. Payment must be submitted in the form of check or money order, made payable to the Commonwealth of Massachusetts. Do NOT mail cash. 

Write the license number on the front of the check or money order.
