To apply, you must complete the application for a non-certified record of birth prior to adoption.

How to apply

The Registry of Vital Records accepts walk-in customers during business hours.

Payment may be made by cash or check. Please make checks payable to Commonwealth of Massachusetts.

Send your notarized and completed application, along with a check payable to Commonwealth of Massachusetts, to:

Please complete your application carefully. Incomplete applications will not be accepted.

Next steps

What do I receive?

When we receive your application, we will search our files. If a birth record from before an adoption is found, you will be sent:

  • A certified mail return receipt
  • A copy of that birth record
  • The legal parents listed on the record at time of birth
  • Any relevant amendments (if applicable)

Note: Only completed applications will be processed. Applications will be processed in the order that they are received.

More info

When trying to obtain a pre-adoption certificate, it is important to remember:

  • RVRS only holds adoption birth records from 1926 to the present.
  • RVRS does not have birth records for any person born outside of Massachusetts.
  • If your application can not be processed for any reason, it will be returned to you with a refund.
  • If you do not sign for the record that is sent to you by certified mail, and the record is returned to RVRS, you will not be issued a refund.
