What you need   for Apply for an Apprentice Embalmer License

To complete an application, enter all required information and submit the following documents: 

  • A 2x2 passport-style photo. 
  • A copy of applicant's High School Diploma or equivalent.
  • A copy of OSHA Certificate showing class was completed within 30 days prior to applying.
  • Apprentice Affidavit Form.
  • Notarized CORI Acknowledgement Form.

Fees   for Apply for an Apprentice Embalmer License

Payments may be made with a check or money order. Please make checks or money orders payable to The Commonwealth of Massachusetts. Fees are non-refundable and non-transferable.
