What You Need for a License Application
It is recommended that you contact the OOA prior to submitting your application.
- Complete the Outdoor Advertising License Application
- Completed W-9 form with original signature
- A Certificate of Good Standing And/Or Tax Compliance from the MA Department of Revenue (For Corporations and Limited Liability Companies Only)
Fees
A new license fee in the amount of $1000.00 must accompany this application.
