What you need

Before applying for certification in Massachusetts:

  • You must have obtained DBE certification in your home state.
  • Provide a home state certification letter (or print-out from the Certified Directory) .
  • Complete and submit all forms below.
  • A copy of your Personal Narrative (PN) submitted to your home state (JOC)
  • A copy of your recent Personal Net Worth Statement (PNW) submitted to your home state (JOC)
  • A copy of your reevaluation letter from your home state (JOC)
