How to apply Apply for or renew accreditation as an EMS training institution
- Download and complete the Application for Accreditation for Initial EMT Training Institution (Full Application)
- Download and complete the Criminal Offender Record Information (CORI) Acknowledgment Form.
- Submitted for all individuals with a significant financial or management interest in the training entity. This includes owners, program directors, medical director, instructors/faculty, instructor aides, and/or clinical and field coordinators.
- Due to updated Criminal History Systems Board regulations, forms must be notarized and include the signature of a Notary Public with the notary stamp or seal (see page 3). Notaries are commonly found at banks.
- Mail completed application and any required attachments to:
