3. Obtain a Transmittal Form & Number

   A unique Transmittal Number is needed for each application you submit to MassDEP.

   Note: There is no fee associated with this approval.

4. Mail Your Application

   Submit a copy of your Transmittal Form to:

   Department of Environmental Protection

   Boston, MA 02211

   Submit your completed, signed application, and Transmittal Form in duplicate to:

   MassDEP Regional Office
   Attention: Air Permit Section Chief
