The site or location where the alleged misconduct occurred

The MassDEP Release Tracking Number (RTN) of the site if it has one

Description of the alleged misconduct, any supporting documents and the names of any witnesses
How to file File a complaint against an LSP
You can file your complaint with the LSP Board by mail. After collecting the information mentioned above, you can complete the complaint form. When the form is complete, you can mail it to:

Next steps for File a complaint against an LSP
After the LSP Board receives your complaint
1. Each complaint is assigned to a staff investigator who determines whether the LSP Board has jurisdiction over the matter.
2. If the complaint falls within the LSP Board’s jurisdiction, the LSP Board reviews the complaint and decides whether further investigation is warranted.
3. If the LSP Board determines that an investigation is warranted then a Complaint Review Team (CRT) comprised of two Board members and a staff attorney conduct an investigation.
4. Based on the results of the of the CRT’s investigation, the LSP Board determines whether discipline is warranted.
