File employee organization reports (Forms 1 and 2)

Pursuant to M.G.L. c. 150E, Sections 13 and 14, all employee organizations must file these reports. The DLR will not process a union's ULP charge until the reports are filed and current.

  • Employee organization's name
  • The name and address of its secretary or other officer to whom notices may be sent
  • The date of its organization
  • Affiliations, if any, with other organizations

Form 2:

  • Balance sheet
  • Operating statement
