The Details of How to Apply for Home Care Assistance Program (HCAP)

Depending on funding, HCAP may have a waitlist for eligible consumers or may establish open enrollment periods during which new referrals will be accepted.
At all times, other comparable benefits for which the applicant may be eligible will be investigated.
MRC Connect staff are responsible for collecting: financial documents, signed release forms, and medical documentation before making a determination of eligibility.
Once determined eligible, MRC Connect will forward your information to MRC HCAP for assignment to a Case Manager, your Case Manager will arrange for an in-home assessment to make a recommendation for a service plan which will include the number of hours of services, and specific tasks needed.
It can take up to 3-4 months to process an application, determine eligibility and arrange for services.
