What you need
- Completed application
- Application fee amount of $600.00
- 2x2 passport style color photo
- White or off-white background
- Official transcripts from a bachelor’s program or equivalent and doctor of naturopathic medicine or doctor of naturopathy program/degree.
- Transcripts must be complete and state the degree and date conferred.
- The educational institution must send transcripts directly to the Board.
- Curriculum Vitae (CV) with Naturopathy practice history
- Two letters of recommendation
- At least one shall be from a licensed N.D., M.D., or D.O.
- Certificates of Completion for fifty (50) hours of continuing education (CE) obtained during the past two years
- An essay describing the Applicant’s scope of practice
- Note: Essays must be 500 words or less
- A National Practitioner Data Bank-Healthcare Integrity and Protection Data Bank Self-Query. To request a Self-Query, please contact the National Practitioner Data Bank. The Self-Query must be submitted to the Board without any modifications.

If applicable
- Official verification of licensure from any state or jurisdiction in which you now or have previously held any professional license.
- The state or other jurisdictions must send verifications directly to the Board.”
- If you answer "yes" to any of the Professional and Criminal History Questions:
- Submit a personal statement explaining the circumstances.
- Submit all supporting documentation (police reports, court documents, disciplinary consent agreements, etc.)
Fees
- All fees are non-refundable and non-transferable
- Check or money orders payable to the Commonwealth of Massachusetts.
- Cash or foreign currency is not accepted.

- All requested information must be provided; failure to provide requested information may result in a delay in processing of an application.
- Do not leave blanks. If you are unable to provide the requested information, attach a separate sheet with an explanation.
- Keep a copy of your complete application for your records. The Board is not able to provide copies of your application.
- The address printed on your license is a public record that is available to anyone who requests it. If you are using your home address, you may wish to consider changing this to an office address. You may request address change online at the Board’s website, or you may get a form online to submit to the Board’s office.
