Contracts are only provided to instructors who have been certified by the MPTC to instruct in a specific topic area and are teaching for the MPTC. The State Fiscal Contract must be approved prior to commencement of instructional services.

What you need   for Receive payment for Instructional Services (Instructor Contract)

Instructors who have been certified by the MPTC to instruct in a specific topic area and are interested in teaching on behalf of the MPTC should complete the MPTC Instructor Fiscal Contract online, sign and scan to mptcinstructorcontract@mass.gov or mail an original to MPTC H.Q.

(NOTE: Existing contracted instructors should log on to Acadis to renew their Certification and go to Next Step #2 to renew their contract prior to its expiration.)

  • Download the required forms and complete online.
  • Print completed forms.
  • Scan to mptcinstructorcontract@mass.gov or mail the original, signed MPTC State Fiscal Instructor Contract to MPTC H.Q., 42 Thomas Patten Dr., Randolph, MA 02368.
  • The instructor's contract must be approved prior to commence instructional services to receive compensation for the services rendered. You will be notified when you are officially contracted to teach with the MPTC. Plan on up to two months for the process to be completed. 

Acceptable Forms of Signature

All CTR forms, including the Standard Contract Form, W-9s, Electronic Funds Transfer (EFT) forms, ISAs, and other CTR-issued documents and forms, or documents related to state finance and within the statutory area of authority or control of CTR (i.e., contracts, payrolls, and related supporting documentation), CTR will accept signatures executed by an authorized signatory in any of the following ways:

1. Traditional “wet signature” (ink on paper).

2. Electronic signature that is either.

a. Hand drawn using a mouse or finger if working from a touch screen device.

b. An uploaded picture of the signatory’s hand drawn signature.

3. Electronic signatures affixed using a digital tool such as Adobe Sign or DocuSign.

  • If using an electronic signature, the signature must be visible, include the signatory’s name and title, and must be accompanied by a signature date.
  • Please be advised that typed text of a name not generated by a digital tool such as Adobe Sign or DocuSign, even in computer-generated cursive script, or an electronic symbol, are not acceptable forms of electronic signature.

  2. Contract Renewal (Existing Instructor)

    Existing instructors contracted (paid) by MPTC for instructional services are required to:

      • Download and complete the Instructor Contract Renewal form prior to its expiration
      • Scan to mptcinstructorcontract@mass.gov or Mail the original, signed MPTC State Fiscal Instructor Contract to MPTC H.Q
      • Existing instructor's contract must be renewed prior to commence instructional services to receive compensation for the services rendered.
