Report a case of Tuberculosis Disease or Latent TB Infection

Both active TB disease and Latent TB Infection are reportable to the Massachusetts Department of Public Health.

What you need

Active Tuberculosis Disease

If you are a provider and have a patient that you strongly suspect may have active TB disease, or you have received laboratory confirmation of Active TB disease in one of your patients, you must immediately report to DPH. Submit an electronic Case Reporting Form at this link.  Or scroll to the bottom of this page to download a Case Report Form and fax the form to (617) 887-8789.

Latent TB Infection

If you are a provider that has administered a Tuberculin Skin Test (TST) or an Interferon Gamma Release Assay (IGRA) where the result was positive, you must complete a Latent Tuberculosis Infection reporting form.  This form is only available online.  Submit an electronic Case Reporting Form at this link.

Active Tuberculosis Disease

Report Active Tuberculosis via an electronic Case Reporting Form.  Select the Active Tuberculosis option at the top of the electronic Case Reporting Form.

Latent TB Infection

Report Latent TB Infection via an electronic Case Reporting Form.  Select the Latent Tuberculosis option at the top of the electronic Case Reporting Form.

Fax the completed Active TB Case Report Form to: (617) 887-8789.

Latent TB Infection

Online only.
