Please note these instructions are for Community EMS, Mobile Integrated Health Care (MIH), and MIH with ED Avoidance (EDA) programs to self-report serious incidents.

If you want to file a complaint about care you or someone else received by personnel as part of a Community EMS, MIH, or MIH with EDA program, please follow instructions on the File a Community EMS, MIH, or MIH with EDA complaint page.

  • Your program name and address
  • Date and location of incident
  • Names and certification numbers (if applicable) of program staff involved in incident
  • Patient care report or encounter record(s) number(s)
  • Descriptive information about the nature of the incident and corrective action taken

How to report

  1. Download and complete the MIH and Community EMS Programs Serious Incident Report Form.
  2. Fax the form to (617) 887-8751. Do not email this form.
